Create the form
- Open the Clayful Consent Google Form
- Click the three dots in the upper right hand corner
- Click Make a copy

Add your District and School as the title of the Google Form in the upper left hand corner and remove the brackets
Update your student grade(s) on the form
- If your school supports different grades from what is currently available, you can add a grade by clicking add “Other”
- You can remove a grade by clicking the X next to the listed grade
Sharing your consent form with families

When the form is updated and ready to send to families, click Share in the upper right corner
- Update the General access settings to
- Anyone with the link
- Viewer
- Click Copy link
- Paste the link wherever you’d like to share the form

Tracking and sharing form submissions with Clayful
- Click on the Responses tab
- Click Link to Sheets
- Select Create a new Sheet and edit your District and School (if needed)
- Click the Share button in the the top right corner
- Click the Send button in the bottom right corner
Setup email notifications when a form is completed (optional)
- Click on the Responses tab
- Click the three dots to the right of “Link to Sheets”
- Click Get email notifications for new responses
